I was about to but thought I’d do a last minute check…seems to be fixed now.
I’m having this same issue.
This hasn’t happened before on my other sites. Today I made a new site and added another verified recipient.

I tested on different devices and networks.
Your form is likely not setup correctly, you either haven’t activated the Smart Forms switch or haven’t selected a recipient.
