Can anyone inform me why a smart form wont work if site uploaded by FileZilla
its (https://www.dyfipetcremations.co.uk)
It works fine in preview mode so I suspect its a setting in Filezilla causing the issue.
I cannot get the upload to work in Bootstrap Studios native system which is why I am exporting it and using FileZilla

Yeah, looks like you are missing the smart-forms.min.js file. That should be exported with your site, if not here is the contents of that file, from one of my sites.
In theory the JS file should be located in your assets folder > JS .smart-forms.min.js
